Staff cannot open Outlook on the web, messages remain in the Outbox, email queues keep growing or servers recover after a restart only to fail again later.

For a 50–500-person organisation, this quickly becomes more than an IT problem. Customer responses are delayed, invoices do not leave, executives lose confidence and the IT team spends hours restarting services without knowing whether the underlying issue is fixed.

What is happening at a high level

Exchange Server is the software that runs email, calendars and shared mailboxes inside your own infrastructure. Unlike Exchange Online, where Microsoft operates the underlying servers, an on-premises deployment depends on your Windows servers, storage, network, security tools and Exchange configuration all working together.

    EXCHANGE SERVER SUPPORT

    Experiencing Exchange Server Issues After
    the October 2026 Updates?

    CPI Consulting helps Australian businesses
    troubleshoot Exchange Server issues,
    assess security updates, and plan
    migrations to Exchange Online.

      Exchange Server 2016/2019 troubleshooting

      Security patch and configuration reviews

      Exchange Online migration planning

    Contact CPI Consulting →

    Australian-based Microsoft cloud consultancy

The instability reported after 13 October 2026 is associated with Windows Server security updates, not the October Exchange Server security update itself. It can affect Exchange Server 2016, Exchange Server 2019 and Exchange Server Subscription Edition.

Affected systems can exhaust saved communication contexts used by Windows Remote Procedure Call, or RPC. In plain English, RPC helps Exchange components and Windows services communicate. Under sustained load, the affected servers can run out of room to maintain those conversations, causing mail flow, database and web access failures.

The relevant Windows Server updates include KB5126646 for Windows Server 2025, KB5126655 for Windows Server 2022, KB5126651 for Windows Server 2019 and KB5126868 for Windows Server 2016. Not every server will fail, and higher-load environments may experience symptoms sooner.

Immediate Exchange Server triage steps

1. Confirm the business impact before changing anything

Start by establishing what is actually failing. Check whether the problem affects all staff, one office, Outlook on the web, desktop Outlook, inbound email, outbound email or internal messages only.

Ask the service desk to record the first known failure time and avoid repeatedly rebooting servers without collecting evidence. A restart may temporarily restore service, but it can also remove valuable clues and create the impression that the incident is resolved.

Test sending an internal message and a message to an external address.

Confirm whether external messages are arriving.

Check whether Outlook on the web accepts sign-ins.

Record queue sizes, database status and affected servers.

Confirm whether a Database Availability Group, or DAG, still has quorum. A DAG keeps mailbox database copies across multiple servers for resilience.

2. Confirm which updates were installed

Check the Windows update history and compare installation times with the start of the incident. The following PowerShell command provides a quick view of recently installed updates:

Get-HotFix | Sort-Object InstalledOn -Descending | Select-Object -First 15

Do not assume the Exchange security update caused the problem simply because it was installed during the same maintenance window. Removing the wrong update can extend the outage and leave a business-facing email server exposed to known security threats.

This is also why staged testing matters. Our earlier guide on why mid-market patch validation cannot be optional explains how a small pre-production group can prevent a routine update from becoming a company-wide incident.

3. Apply the recommended Windows RPC mitigation

The current recommendation is not to uninstall the October Windows Server security updates. Instead, set a higher limit for the RPC Endpoint Mapper saved context capacity and then restart the Exchange server.

Run the following command from an elevated Command Prompt on each affected Exchange server:

reg add "HKLM\\SOFTWARE\\Microsoft\\Rpc" /v EpmapSavedContextCap /t REG_DWORD /d 65536 /f

Reboot the server through your normal change process. In a multi-server environment, work on one server at a time so that functioning systems can continue handling email.

Monitor the server after the restart rather than treating the reboot as proof of recovery. If the symptoms return, escalate through Microsoft support or an experienced Exchange specialist before increasing the value further.

4. Validate mail flow and Exchange health

Once the server returns, confirm that the Exchange services are running and queues are reducing. Growing queues can mean messages are entering the system faster than Exchange can process or deliver them.

Test-ServiceHealth
Get-Queue
Get-MailboxDatabaseCopyStatus *

The first command checks whether required Exchange services are available. The second displays mail queues, while the third checks database copies in a DAG environment.

Also run the Microsoft Exchange Health Checker. This identifies outdated builds, missing security actions, configuration risks and unsupported software combinations that may have contributed to the incident.

5. Check whether the environment was already fragile

An update often exposes an existing weakness rather than creating every problem from scratch. Review storage capacity, server memory, backup status, database replication, certificate expiry dates and monitoring alerts.

Exchange Server 2016 and 2019 reached end of support on 14 October 2025. Organisations still using them require special Extended Security Updates to receive eligible security fixes, and that arrangement should be treated as a temporary bridge rather than a long-term operating model.

A realistic mid-market scenario

Consider a 200-person professional services firm with two Exchange servers. After the October Windows updates, staff can send email in the morning, but queues build during the day and Outlook on the web begins failing. Restarting the busiest server restores service for several hours.

The immediate priority is to apply the RPC mitigation in a controlled sequence, verify database health and watch the queues. The larger question is whether the firm should continue paying for server hardware, backup storage, Exchange maintenance, after-hours patching and specialist incident support.

If recurring incidents consume 15 to 20 technical hours each month, the cost is not limited to IT labour. Delayed proposals, missed customer messages and disrupted payroll or invoicing can easily become the larger expense.

When Exchange Online becomes the lower-risk option

Moving to Exchange Online means Microsoft operates the email servers as part of Microsoft 365. Your organisation still controls users, security policies, retention and access, but no longer maintains the underlying Exchange databases, operating systems or physical infrastructure.

A move deserves serious consideration when several of the following are true:

You are still running Exchange Server 2016 or 2019.

Email outages are becoming more frequent or difficult to diagnose.

You do not have a separate environment for testing updates.

Only one or two people understand the Exchange configuration.

Hardware, storage or backup infrastructure needs replacement.

Cyber insurance or customer contracts require stronger controls.

Your organisation already pays for Microsoft 365 licensing that includes Exchange Online.

Exchange Online does not remove every risk. Identity security becomes especially important because a stolen password can provide access from anywhere. Our Microsoft Entra ID security review covers the controls that protect Microsoft 365 accounts, while the Microsoft 365 tenant hardening roadmap explains how to reduce risk without making work unnecessarily difficult.

When staying on-premises may still be justified

Some organisations have applications that depend on local Exchange, strict operational requirements or sites with unreliable internet connectivity. In those cases, Exchange Server Subscription Edition may remain appropriate, provided it is properly supported, monitored and maintained.

The important point is to make that decision deliberately. Keeping Exchange because migration has been postponed for several years is not a strategy, especially when unsupported software can work against Essential 8, the Australian government’s cybersecurity framework that many organisations use to demonstrate reasonable security practices.

Plan the move without creating another outage

For most mid-market organisations, a staged hybrid migration is safer than a rushed cutover. Hybrid means the on-premises and online systems work together while mailboxes are moved in controlled groups.

Assess mailbox sizes, shared mailboxes, archives and application dependencies.

Review identity, licensing, retention and backup requirements.

Test a small group representing different departments and devices.

Move mailboxes in batches with clear rollback and communication plans.

Validate mail flow, mobile access, shared calendars and line-of-business applications.

Remove or reduce the on-premises footprint only after dependencies are confirmed.

Microsoft 365 retention and recovery settings should also be reviewed before migration. They are not automatically the same as a traditional backup. If email recovery is a concern, our guide to recovering deleted or lost Exchange Online email explains one available process.

Stabilise today and make a deliberate decision

The October 2026 incident does not mean every organisation must immediately abandon Exchange Server. It does show how quickly one Windows update can affect email, database availability and business operations when your organisation carries the full infrastructure burden.

CloudPro Inc combines more than 20 years of enterprise IT experience with practical Exchange, Microsoft 365, Azure and cybersecurity expertise. As a Melbourne-based Microsoft Partner, we help organisations across Australia assess incidents, stabilise environments and compare the real cost of Exchange Server with Exchange Online.

If you are unsure whether the October updates are behind your Exchange instability, or whether maintaining on-premises email is costing more than it should, we are happy to take a practical look at the environment with you — no strings attached.

Please enable JavaScript in your browser to complete this form.
Please enable JavaScript in your browser to complete this form.

Full Name *

Email *

Company Name *

   or Comment Name

Comment or Message *

    ( function() {
     const style = document.createElement( 'style' );
     style.appendChild( document.createTextNode( '#wpforms-53280-field_1-container { position: absolute !important;
